The best way to form a team is to find 5 like-minded individuals, bonus points if they can play the respective classes needed, and just start from there. Personally, the traits that stand out the most to me are loyalty and dedication to the game. As long as people are always willing to improve their team and are always going to be dedicated to each other, then the sky's the limit. This is the foundation that I put my team together on and I truly believe that given enough time and effort, we'll be a top CEVO-P competitive team. Obviously, an above average level of skill is necessary to start out given the playing field but even that can be taught.
If you look at some of the better teams today, these are things they lack and they're constantly hurt for it. They see a team as a temporary entity. There's no unity, just people playing together and carrying out their roles as individuals. Find people you can keep a close bond with and you'll go far.
