I know the guy you're talking about. I spectated him just to make sure he wasn't hacking. He missed more than he got headshots. My accuracy might be similar.
When you're only going for the head, you either miss or you get headshots (or you miss and get a body shot and then appologize "cuz you ain't no bodyshottin noobie"). This is why I have such a high headshot percentage when I play. I think last night I had 56 kills with 51 of them being headshots.
It's fine to be suspicious of people who are getting only headshots, but to call them out as a hacker or aimbotter just because they're owning you is silly. You're not the best player in the game. Just because you cannot comprehend how someone gets 90% headshots doesn't mean it's not possible.
To answer your questions:
1) Yes someone like this is possibly aimbotting. Usually, the more people that are bitching about the player the more likely the player is hacking. Call an admin in to check if you're not sure. One of these days, spectator will be enabled so that you can watch for yourself. Yes it's bad to etiquette to call them out on it because you're probably wrong.
2) Practice. ctf_2fort Instant respawn is a good place to do it because even if you die, you don't have to wait to try again. Also, the guy you're talking about in particular was especially hard to hit because he kept going in and out between the poles on the battlement.
